Primary Care Network
Healthier Neighbourhoods PCN
For the communities we serve on the Wirral:
Birkenhead, Beechwood, Noctorum, Prenton and Woodchurch
Healthier Neighbourhoods PCN is a new Primary Care Network; a collective of 3 GP practices working together across 6 locations for the benefit of the communities and patients we serve.
Our practices include Paxton Medical Group (including Claughton, Townfield, and Fender Way surgeries), Heatherlands Medical Centre, and Prenton & Woodchurch Medical Centre.
The patient benefits to being part of our PCN include:
- Services will be delivered in our patients’ local surgeries rather than elsewhere on the Wirral.
- Our patients will be able to access enhanced services such as first contact physiotherapy, health coaching, and social prescribers closer to home.
- Patients will benefit from additional GP appointments, including evenings and weekends.
Our vision as a PCN is to address health inequalities within our communities, improving the health and lives of over 30,000 patients.
